Insurance reimbursement write-up for counseling sessions for 1 month ($10 base). This is a flat fee item regardless of the number of visits that have occurred in the covered month. Once payment is completed for the program, your Counselor will send you a write-up with the necessary items needed to send into your health insurance agency to request reimbursement via email.
